How do you make the handoff from an AI agent to a human feel smooth?

We use an AI agent for first-line support, and the weakest moment is when it has to pass a customer to a person. Customers end up repeating everything, or the human gets a wall of chat log with no idea what matters. Which tools or setups actually handle this handoff well?

A good handoff carries a short summary written for the person taking over: who the customer is, what they want, what the agent already tried, what it promised, and why it is handing off. If the human has to scroll the transcript to find that, the handoff has already failed.
